What happened
A US Tomahawk missile strike on February 28, 2026, hit the Shajarah Tayyebeh elementary school in Minab, Iran, killing at least 175 people, mostly children. An ongoing military investigation, citing US officials, determined the strike resulted from outdated data used by US Central Command (CENTCOM) to generate target coordinates. The Defense Intelligence Agency (DIA) provided the coordinates for the school, which had been repurposed a decade prior from a military complex. Officials confirmed AI models, including Anthropic's Claude AI, did not directly influence the targeting for this specific incident.
Why it matters
Intelligence failures, not AI systems, remain a critical risk in military operations. Procurement teams and security architects evaluating AI integration must prioritise the accuracy and recency of underlying data feeds over model capabilities. While the DoD recently deployed AI for military strikes, this event demonstrates that even advanced AI cannot compensate for fundamental human process flaws or outdated intelligence, directly impacting operational reliability and accountability.
